Some Important Things to Remember:

  • You will need to bring your NDIS plan with you on the day of your initial appointment.
  • As required by the NDIA, a service agreement will need to be made between the participant and provider before services can commence. Your AEDC practitioner will help with making a service agreement.

  • When making a service agreement, please keep in mind that the NDIA will often request a NDIS report at the end of your plan. The fee for a NDIS report from AEDC is $360.00 (2 hours report writing).
  • Please give your practitioner a minimum of 14 days’ notice if a NDIS report is required.
  • If a participant makes a short-notice cancellation, which is after 3pm the day before the service, AEDC may charge up to 90% of the agreed price for the cancelled appointment.
  • Please inform AEDC immediately if the participant’s NDIS plan is suspended or replaced by a new NDIS plan or the participant stops being a participant in the NDIS.
  • School visits and other out call services will incur an additional travel fee of $100.00 if within 10km of the clinic. For out calls further than 10km, please speak with our reception for further details.
